We understand that the decision to come forward as a Whistle Blower, with information about illegal activity is not one taken lightly and we assure that your identity will stay protected. By making use of the “Unique Code Name”, NERMF enable you to stay anonymous and still be able to request feedback or divulge further information regarding the initial report. You are welcome to stay anonymous although we found that many “reporters” want to be involved in the follow up processes and/or investigations.

By being here means you took the first step in doing the right thing in reporting something that could hurt you, your company, your client your service provider or just “somebody”. We applaud you for being here.

The report must contain as much information as is readily available by following the questions-

A) Who?

B) Where?

C) When?

D) What?

E) Why?

F) How?

Please complete the form with as much detail as possible.

A) Who?
a. The name(s) of any person or institution involved in the activity and a very good description if needed.
b. The name(s) of any other institution or person involved in the activity.
c. Any information regarding the address and contact numbers or places of the person or entity.
d. Any numbers of any accounts, registration numbers, Identity numbers or other identifying numbers available.
e. Any other information you can think of to help us identify or locate this person or entity like country of residence, occupation, type of business conducted by person or entity.

B) Where?

a. Explain where the activity happened with as much details as possible to help us to find the exact spot or place.

Just for clarity on our side, please enter the Suburb, Town or Area where the activity did occur or are going to occur.

C) When?
a. The date and time of the illegal activity, or, in the case of a series or ongoing activities the period over which the activities transpired.

D) What?
a. The type of activity.
b. Is there any property involved?
c. The amount or the value if applicable.
d. What documentary proof is available in respect of the activity concerning?
e. In case of suspicious or unusual activities, please include the reason why it is deemed to be suspicious or unusual

E) Why?
a. We do not always know why people do certain things, but if there are anything, please tell us.
b. Any remarks, comments or explanations which the person conducting the activity may have made or given.

F) How?
a. How did it happen?
b. The method in which the activity was conducted.

G) Other Actions taken?
a. Please indicate what other action(s) has been taken in connection with the activity for example did you contact the SAPS, Child line or any other hotline or reporting procedure?

H) Any other information?
a. Please inform us of any other relevant information that you may have or anything else you want to tell us.
